We look forward to working with you and your family to provide a quality education for your child. Before we can move on with the fun stuff… we have to get past the paperwork. As you prepare to register your child for enrollment, please take some time to read the following information explaining enrollment procedures.

Only the student’s parent or legal guardian may register them to attend school. If you are not a parent, listed on the birth certificate, guardianship papers must be presented at the time of enrollment. A picture ID will be required.

Step-parents may NOT register a child unless they have papers naming them as a legal custodian.

What You Need to Bring to Register Your Child

Parents or legal guardians must bring a picture ID
Proof of residency

A signed residential lease agreement – we must see a signed original,
or A non-contingent sales contract on a home – we will accept a HUD-1 statement (closing statement)
or a warranty deed or security deed provided that the owner’s name and a street address are evident on the same page. *If the legal description is all that is evident on the deed, two utility bills will be required.
and A current utility bill – gas, water or electric – not phone or cable

If you do not own the home in which you are living , or are not listed on the title, please stop by the school to get a Gwinnett County Residency Affidavit which must be completed, notarized and submitted with the same documentation required above.

If the house is in your spouse’s name , we will need a copy of the marriage certificate showing the relationship between the home owner and the enrolling parent.

Georgia Form 3231 (immunization) and Georgia Form 3300 (vision/hearing/dental certificate)

These forms may be obtained from the student’s personal physician or a Gwinnett County Health Department
If a student has been in Georgia schools, the last school attended should have these forms on file….but YOU need to request them from the school and bring them with you. By law, we cannot admit a student into our school without immunization forms in hand.
If you are transferring from another Gwinnett County school, the indication on the withdrawal form stating the file is complete is sufficient. We will get the original record from the previous Gwinnett County School.
Based on DHR regulations, the Georgia 3231 form must be dated 2/17/07 or after

Copy of the most recent report card
Social Security Card (optional – waiver may be signed)
Birth Certificate – we must view the original – we will make a copy for our file
Withdrawal Form from the school the student is leaving
IEP if the student is in Special Education
Gifted eligibility records if the student is in the gifted program
Discipline summary if applicable and coming from another Georgia school
